The Donate Life license plate highlights the need for more organ, tissue and eye donors. More than 3,000 of your Wisconsin neighbors are waiting for an organ, tissue or eye donation. Any of us could suffer an injury or illness that requires a transplant, making the need for registered donors greater than ever before. Transplants offer patients a new chance at healthy and productive lives, returning them to their families, friends and communities.
This license plate includes the Donate Life symbol, which represents the work being done to save and improve lives through organ, tissue and eye donation.
Donate Life Wisconsin (DLW) is a non-profit alliance of organizations and caring individuals across the state committed to this work. DLW motivates people to become registered donors through awareness, education, registration, advocacy and support of donor families.

Personalized plate search to find out if your choice for a personalized license plate is available. WisDOT may refuse to issue, or may recall after issuance, a request that may be offensive to good taste or decency, misleading or conflicts with any other license plate. Please allow an additional 4 weeks for delivery of a personalized plate.
DMV service centers do not issue Donate Life license plates.
