Not all DMV customer service centers provide license plates. Save yourself time and gas by checking the plate availability tool below to see which customer service centers have the plates you need. Note: All license plates are available by mail.

Some regular and special plates can only be applied for through the mail. Several special plates (those that highlight within the plate availability tool below) can be applied for either through the mail or at a customer service center and then plates will be sent through the mail. You will receive a temporary plate if needed.

All customer service centers (except the express center on Odana Road in Madison) issue disabled parking ID cards.

To apply for license plates by mail, complete the MV1 PDF (684 KB) form and mail to:

Instructions

  1. Select a plate type from the dropdown menu at the top of the tool.
  2. Read the instructions associated with the selected plate type.
  3. DMV service centers that provide the plate will highlight. Click any of the highlighted service centers for more information.

Note: The license plate availability tool requires Flash player 9 or newer. You can also review the plate availability chart PDF as a non-Flash substitute.
