Becoming an organ and tissue donor

You can help save and improve lives by registering to be an organ and tissue donor when you renew your driver license or ID card. People of all ages can donate regardless of health history.

Complete form MV3001 (561 KB) - Wisconsin Driver License Application or MV3004 (405 KB) - Wisconsin Identification Card (ID) Application, and check the box indicating you wish to register as a donor. Registering means that you authorize the donation of your organs, tissues and eyes upon your death. Your gift will be used to save and improve lives through transplantation, therapy, research or education. If you are 18 years old or older, your decision may not be overridden by your family or any other person, but it's important to share your decision with your family.

You can also register online at DonorRegistry.wisconsin.gov (or remove your name from the registry if desired). The Wisconsin donor registry is maintained by the Wisconsin Department of Health Services.
